ABOUT YOUR VISIT

We want your first visit to be as pleasant as possible and we want you to know what to expect. We want our patients to be treated in a warm and compassionate manner. When you visit us for the first time, we will take your medical history and do a medical evaluation. Please bring any medicines you are taking and any pertinent cardiac tests your family physician has conducted. This may reduce the amount of testing we need to perform.

Please bring your insurance cards. If you have an insurance plan that requires a referral form, please be sure that you bring the referral from your primary physician on the day of the appointment.
